Insurance Plans
We participate in most major health insurance plans. Please check with your insurance provider at the outset of treatment to ensure that you are covered for your treatment (and at what level) and to be sure that we are listed as a provider in your plan.
Patient Responsibility
It is the patient’s responsibility to check their insurance plan coverage. To avoid unpleasant and expensive surprises, we strongly encourage our patients to carefully research the specific provisions of their health insurance coverage.
Insurers typically deny coverage if you do not meet their provisions. It is the patient’s responsibility to know the specific requirements of their coverage, and to inform our office of all special requirements.
When you schedule your first appointment at Diamond Lake Clinic you will be asked to provide your insurance information over the phone, along with other identifying information. This information allows our billing and scheduling departments the opportunity to prepare your new patient file prior to your appointment. The day of your appointment you will be provided with additional intake forms that will need to be filled out. Please arrive 15 minutes early to fill these out. Please make sure to bring a valid ID and your insurance card. Without these items we cannot proceed with your appointment and you will need to reschedule.
Payment is due at the time services are rendered. Your payment may consist of an insurance deductible, a co-payment, co-insurance, or full payment for uninsured patients or those services not covered by an insurance plan.
It is important that you bring your insurance card to every appointment. If you change insurance companies or change your address, phone number, etc., please contact our office as soon as possible so we can make the necessary changes to your file. Patients are responsible for all bills denied by their insurance carrier.
