Our practitioners see patients on an appointment basis.
If you are experiencing problems, please call during our office hours on weekdays. We will arrange for you to see someone as soon as possible. We offer appointments 8:00 am 7:00pm, Monday – Friday, although our office hours are 8:30am – 4:00pm.
If you are unable to keep your appointment, please let us know as soon as possible. Our practitioners make every effort to keep their appointments on schedule. Emergencies do arise, however, and sometimes a delay is unavoidable. When this happens, we do all we can to minimize the inconvenience. If an emergency occurs and you need to contact us during office hours, call us.
Note: We will require a copy of your valid ID or driver’s license, as well as a copy of the front and back of your insurance card sent to our secure email info@diamondlakeclinic.org. The day of your appointment, you will be provided with additional intake forms that will need to be filled out. Please arrive 15 minutes early to fill these out. Please make sure to bring a valid ID and your insurance card. Without these items we cannot proceed with your appointment and you will need to reschedule.
Due to the COVID-19 situation: We are currently scheduling everyone to do Telemedicine visits (via computer, phone, tablet, ipad, etc) until further notice. The clinic is limiting in-person visits on a strict case by case basis, including TMS sessions.
When you schedule your first appointment at Diamond Lake Clinic you will be asked to provide your insurance information over the phone, along with other identifying information. This information allows our billing and scheduling departments the opportunity to prepare your new patient file prior to your appointment.
